ABSTRACT

BACKGROUND: Neurocysticercosis is the most prevalent parasitic disease of the central nervous system in Chile, where sporadic cases are reported, without information about the epidemiology or distribution of the disease. AIM: To identify the main risk zones for cysticercosis in Chile. MATERIAL AND METHODS: Analysis of hospital discharge databases between 2002 and 2019, available at the website of the Chilean Ministry of Health. Cases with B69 code of the tenth international classification of diseases were identified. RESULTS: In the study period, there were 1752 discharges with the diagnoses of neurocysticercosis, ocular cysticercosis and cysticercosis of other sites. The ages of patients ranged from 0 to 89 years with a clustering between 30 and 59 years. Sixty two percent were males. The zone between the regions of Maule and Araucania concentrated 82% of cases. CONCLUSIONS: We identified the zone with the greatest concentration of cysticercosis in Chile, where preventive strategies should eventually be directed.

ABSTRACT The Taenia solium cysticercosis affects millions of people worldwide and is considered a public health problem, especially in developing countries. The diagnosis of neurocysticercosis is complex and involves the analysis of epidemiological, clinical, neuroimaging, and immunological host data. Neurocysticercosis is endemic in Brazil, and is related to the cause of death mainly in the Southeast, South, and Central-West regions. The objective of this study was to determine the seroprevalence of cysticercosis in inhabitants of the city of Jataí, Goiás, in the Central-West region of Brazil from April to August 2012. A total of 529 serum samples were analyzed by enzyme-linked immunosorbent assay for detecting IgG antibodies against T. solium larvae, and Western blotting was used for confirming the diagnosis through the recognition of at least two specific peptides from their serum antibodies. The 351/529 (66.3%) reactive samples were analyzed by enzyme-linked immunosorbent assay and Western blotting confirmed the diagnosis in 73 samples that recognized at least two of the following peptides specific IgG antibodies for cysticercosis: 18, 24, 28-32, 39-42, 47-52, 64-68, and 70 kDa. The seroprevalence of cysticercosis was 13.8% (95% CI 5.9-21.7), demonstrating that the studied area is endemic to this disease.

ABSTRACT

Cisticercose bovina é uma importante doença parasitária de caráter zoonótico, com elevada ocorrência em algumas regiões do Brasil. Considerando a possibilidade de erro na identificação das lesões, bem como a dificuldade de classificação dos cistos e a necessidade de melhorar o diagnóstico, o objetivo desse trabalho foi caracterizar e correlacionar as lesões macroscópicas e microscópicas da cisticercose bovina, além de utilizar a técnica da PCR para auxiliar na identificação do agente. Amostras de lesões císticas e nodulares de bovinos, compatíveis macroscopicamente com cisticercose, foram coletadas em abatedouros frigoríficos do Estado do Rio Grande do Sul. Os cistos foram divididos em três grupos: Grupo 1, cisticercos vivos (viáveis); Grupo 2 (subdividido em 2a e 2b), cisticercos degenerados com potencial escólex viável; e Grupo 3 cisticercos mortos (mineralizados). Após a obtenção das lâminas histológicas dos cisticercos de cada grupo, foi realizada a correlação macroscópica e microscópica. Para a realização da técnica da PCR foram utilizadas lesões císticas de 26 bovinos. Foram analisados cisticercos de 127 bovinos, totalizando 232 cistos. Dos 127, 46 bovinos (36,2%) apresentaram mais de um cisticerco e 81 (63,8%) um cisticerco cada. Em relação a localização anatômica dos cistos, o coração demonstrou o maior envolvimento (55,9%), seguido do músculo masseter (22,8%). Quando houve o envolvimento de dois órgãos em um mesmo bovino, coração e músculo masseter juntos, totalizaram 11 casos (8,6%). De maneira geral a média da frequência de cisticercose foi de 10% a 15% de bovinos acometidos por lote. Entretanto, a média isolada de alguns lotes demonstrou condenações acima de 50%, 80% e 90%. Morfologicamente, os 232 cisticercos foram classificados dentro de três grupos. No Grupo 1, 23 cistos (9,9%) foram considerados como vivos (viáveis), e eram caracterizados por lesões císticas com parede translúcida ou levemente opaca, contendo líquido claro e um ponto esbranquiçado no interior (escólex). Na histologia, os cistos eram compostos por uma membrana de onde invaginava um escólex de Taenia saginata. No segundo grupo (Grupo 2), foram incluídos 156 (67,2%) cisticercos degenerados com potencial escólex viável e macroscopicamente os cistos demonstraram dois padrões morfológicos distintos. No primeiro deles (Grupo 2a), visualizado em 111 casos (71,1%), observaram-se lesões nodulares com aspecto caseoso. Microscopicamente, os cistos caracterizavam-se por formações nodulares compostas por área central contendo escólex e membrana, ambos degenerados, e necrose caseosa. No segundo padrão (Grupo 2b), observado em 45 cisticercos (28,9%), as lesões também eram caseosas, entretanto ao corte os cistos demonstravam na área central um orifício em meio ao material caseoso. Os aspectos microscópicos dos 45 cistos incluídos no segundo padrão macroscópico assemelhavam-se aos cisticercos do primeiro padrão. Entretanto, oito cistos (17,8%) demonstraram somente a membrana parasitária viável e em um cisto notou-se a membrana com o escólex viável. No restante dos 36 cistos (80%), observou-se área central contendo escólex e membrana, ambos degenerados, e necrose caseosa. No terceiro grupo de classificação morfológica dos cisticercos (Grupo 3), foram inseridos os cistos mineralizados (mortos), totalizando 53 cistos (22,9%). O aspecto macroscópico desses cisticercos caracterizava-se por lesões nodulares, amarelas, firmes ao corte, que se fragmentavam. Histologicamente observaram-se formações nodulares com área central de acentuada mineralização, rodeadas por infiltrado inflamatório granulomatoso. Dos 127 bovinos, foi realizado PCR a partir do DNA extraído dos cisticercos de 26 bovinos, no qual 24 foram positivos para cisticercose. Em relação aos dois cisticercos negativos, um deles fazia parte do Grupo 2a e o outro do Grupo 3. A correlação entre os aspectos macroscópicos e microscópicos do segundo padrão morfológico observado dentro do Grupo 2, demonstrou que esse subgrupo representa o maior problema na interpretação, pois alguns cistos apresentaram características de viabilidade. Macroscopicamente esses cisticercos podem ser identificados quando cortados, porque possuem um orifício na área central que pode auxiliar no diagnóstico.(AU)


Bovine cysticercosis is an important zoonotic parasitic disease with high prevalence in several regions of Brazil. Considering the need of improvement of the accuracy of diagnosis of these lesions, as well as the difficulty of classification of the cysts, this study aimed to correlate gross and histopathological changes of bovine cysticercosis and to use polymerase chain reaction (PCR) as an aid in their identification. Cystic and nodular lesions from cattle, grossly compatible with cysticercosis, were sampled in slaughterhouses from Rio Grande do Sul State. Lesions were allotted in one of the following groups. Group 1: viable cysticercus; Group 2 (subdivided 2a e 2b): degenerating cysticercus with a potentially viable scolex; and Group 3: dead cysticercus (mineralized). The gross and microscopic aspects of every cysticerci of each group were compared. Two hundred and thirty two cysts and nodules compatible with cysticercus were sampled from 127 bovine. Twenty six of those lesions were tested with PCR. Out of 127 cattle, 46 (36.2%) had more than one cyst and the remaining 81 (63.8%) had on cyst each. Myocardium was the most frequently involved anatomical site (55.9%), followed by masseter muscle (22.8%). When there was more than one organ involved in the same bovine, myocardium a master muscle sum up 11 cases (8.6%). In general, the average of cysticercosis frequency was 10-15%. However the average in some cattle lots was in excess of 50%, 80% and 90%. Morphologically, 232 cysticerci were classified in three groups. In Group 1, 23 cysticerci (9.9%) were considered viable and were characterized by cysts of translucent or slightly opaque wall, containing clear and a white point (scolex) within the cyst. Histologically, the cysts consisted of a membrane from which a scolex of Taenia saginata invaginated. One hundred and fifty six cysts (67.2%) were allotted in Group 2; grossly these cysts revealed two different morphological patterns. In 111 (71.1%) cases of Group 2 (Group 2a) nodular caseous lesions were observed. Histologically, the cysts were characterized by nodules consisting by a central area containing the scolex and membrane, both degenerated, and caseous necrosis. In the remaining 45 (28.9%) cases of Group 2 (Group 2b), lesions were also caseous; however, at cut surface the cysts had a central hole amidst the caseous material. The microscopic aspect of the 45 cysts included in the second was similar to that of the first pattern. However in eight (17.8%) of the 45 cysts only a viable parasitic membrane was observed and in one cyst the membrane and viable scolex were observed. In the remaining 36 cases (80%), the cysts consisted of a central area containing both degenerated membrane and scolex, and caseous necrosis. In Group 3, 53 dead cysts (mineralized) (22.9%) were found among the total of 232 cysts. The gross aspect of these cysticerci was characterized by yellow form nodules which crumbled when cut. Histologically nodules were observed with marked central area of mineralization surrounded by granulomatous inflammatory response. Twenty four of the twenty cysts examined by PCR were positive for Cysticercus bovis and two of them were negative. One of the negatives was part of Group 2 (degenerated cysts) and the other one of the Group 3 (dead mineralized cysts). The correlation between gross and microscopic aspects of the second morphologic aspect of the Group 2 demonstrated that this subset represents a major complicating factor in interpretation, since a large number of these cysts reveal characteristics of viability. Grossly, these cysticerci might be identified when cut, since a hole in the central area will be observed aiding in recognizing his lesions.(AU)

ABSTRACT

A cisticercose é uma parasitose cosmopolita relevante para a saúde pública, animal e economia. Neste estudo objetivou-se analisar dados epidemiológicos da ocorrência da cisticercose bovina em diferentes continentes. Promoveu-se revisão bibliográfica em artigos científicos, dissertações e teses sobre aspectos epidemiológicos e distribuição espacial da cisticercose bovina. Observa-se que a ocorrência dessa parasitose relaciona-se com diversos fatores de natureza socioeconômica, cultural e práticas de manejo empregues em distintos sistemas de produção animal. Baseada principalmente em dados de inspeção de carnes, a ocorrência dessa parasitose tende a ser subestimada, entretanto, muitos dos casos reportados da parasitose apontam ocorrência de forma endêmica em algumas regiões da África e América Latina. A heterogeneidade na distribuição espacial da parasitose, associada a variações de fontes de infecção dos animais, justifica a necessidade de implantação de práticas de vigilância sanitária e informações adicionais referentes às práticas de criação dos animais aos frigoríficos, possibilitando identificar riscos espidemiologicos e desenvolver medidas alternativas de controle do complexo teníase cisticercose.(AU)

ABSTRACT

A total of 16 Taenia multiceps isolates collected from naturally infected sheep or goats in Gansu Province, China were characterized by sequences of mitochondrial cytochrome c oxidase subunit 1 (cox1) gene. The complete cox1 gene was amplified for individual T. multiceps isolates by PCR, ligated to pMD18T vector, and sequenced. Sequence analysis indicated that out of 16 T. multiceps isolates 10 unique cox1 gene sequences of 1,623 bp were obtained with sequence variation of 0.12-0.68%. The results showed that the cox1 gene sequences were highly conserved among the examined T. multiceps isolates. However, they were quite different from those of the other Taenia species. Phylogenetic analysis based on complete cox1 gene sequences revealed that T. multiceps isolates were composed of 3 genotypes and distinguished from the other Taenia species.

INTRODUCTION: Human serofrequency of antibodies against Taenia solium antigens was determined and risk factors for cysticercosis transmission were identified. METHODS: Individuals (n=878) from periurban and rural locations of Lages, SC, were interviewed to gather demographic, sanitary and health information. Interviews and blood sample collections by finger prick on Whatman filter paper were performed from August 2004 to May 2005. Observation determined that 850 samples were suitable for analysis and were tested by ELISA using vesicular fluid of Taenia crassiceps heterologous antigen. To ensure the reliability of the results, 77 samples of the dried blood were matched with sera. The reactive samples were submitted to a serum confirmatory immunoblot (IB) test using purified Taenia crassiceps glycoproteins. RESULTS: The ELISA results for the dried blood and serum samples were statistically consistent. ELISA was positive in 186 (21.9 percent) out of 850 individuals. A group of 213 individuals were asked to collect vein blood for IB (186 with positive result in ELISA and 27 with inappropriate whole blood samples) and 130 attended the request. The IB was positive in 29 (3.4 percent) out of 850 individuals. A significant correlation (p = 0.0364) was determined among individuals who tested positive in the IB assay who practiced both pig rearing and kitchen gardening. CONCLUSIONS: ELISA with dried blood eluted from filter paper was suitable for cysticercosis population surveys. In Lages, human infection was associated with pig rearing and kitchen gardening. The prevalence index was compatible with other Latin American endemic areas.


INTRODUÇÃO: O primeiro levantamento sobre cisticercose humana e identificação dos fatores de risco associados à transmissão, foram realizados em Lages, SC. MÉTODOS: Oitocentos e setenta e sete voluntários de regiões periurbana e rural foram entrevistados e forneceram informações demográficas e condições sanitárias e de saúde. Amostras de sangue foram coletadas por meio de punção digital em papel filtro entre agosto 2004 e maio 2005. Verificou-se que 850 amostras estavam adequadas para análise. No ELISA, utilizou-se o antígeno heterólogo liquido vesicular de Taenia crassiceps. Para assegurar a confiabilidade dos resultados de ELISA, foram pareadas 77 amostras de soro e sangue eluido do papel filtro. A confirmação do diagnóstico sorológico foi feita por immunoblot (IB) com glicoproteínas purificadas de Taenia crassiceps. RESULTADOS: A reatividade de IgG eluída de sangue em papel filtro mostrou-se compatível com a dos soros correspondentes. A triagem por ELISA de 850 indivíduos revelou 186 (21,9 por cento) positivos. De 213 pessoas convidadas a colher soro para IB (186 ELISA positivo e 27 com amostras de sangue total inadequadas), compareceram 130. O IB foi positivo em 29 (3,4 por cento) de 850 amostras. Houve correlação significativa entre IB positivo e a prática de criação de suínos e de horta caseira (p = 0,0364). CONCLUSÕES: ELISA com sangue total em papel filtro mostrou-se adequado para inquéritos populacionais para cisticercose. A transmissão da cisticercose humana na área estudada mostrou correlação com criação suína domestica e horta caseira. A prevalência obtida foi semelhante à relatada em áreas endêmicas da América Latina.

Este trabalho tem como objetivo, determinar a prevalência de cisticercose bovina em animais abatidos em frigoríficos de Inspeção Estadual do RS. Conscientizar a população de que a erradicação da cisticercose só será possível após serem tomadas sérias medidas preventivas como inspeção sanitária, mudanças de hábitos alimentares, adoção de políticas de melhoramentos de infra-estrutura. Os dados foram obtidos de abates realizados no RS entre 1992 e 2001 fornecidos pelo Ministério da Agricultura, Pecuária e Abastecimento através da Delegacia Federal do RS.

Seven swine were experimentally infected with Taenia solium eggs and blood samples from each animal were periodically collected. At the end of the experiment (t140) the animals did not show clinical aspects of cysticercosis or parasites in tongue inspection. All animals were slaughtered and cut into thin slices in searching for cysts. The number of cysts found in each animal varied from 1 to 85. Enzyme-linked immunosorbent assay (ELISA) tests for antibody (Ab) detection and for antigen (Ag) detection were performed, which presented respectively 71 and 57 percent of positivity. By immunoblot (IB), using 18/14(T. crassiceps Ag) or lentil-lectin-purified glycoproteins from T. solium Ag (LLGP) as Ag, five (71 percent) and six (86 percent) animals were positive, respectively. The association between Ag-ELISA with any IB (18/14 or LLGP) allowed the detection of all animals at 140 days post-experimental infection (days p.e.i.). The use of IB 18/14 combined to the Ag-ELISA allowed the detection of all animals since 70 days p.e.i., and the association between IB LLGP and Ag-ELISA allowed the detection of all animals since 112 days p.e.i. While all animals could be considered healthy by conventional screening tests, the use of immunoassays for detecting Ab and Ag showed better accuracy; therefore it would be more useful than usual clinical examination for screening cysticercosis in slightly infected pigs.

ABSTRACT

O presente estudo teve como objetivos avaliar a prevalência de cisticercose, classificar as etapas evolutivas dos cisticercos encontrados nos encéfalos e coraçöes humanos, diferenciá-las de acordo com os aspectos macro e microscópicos dos processos patológicos gerais e comparar os processos encontrados nos encéfalos e coraçöes. Foram revisados protocolos de autópsias realizadas no Hospital Escola da Faculdade de Medicina do Triângulo Mineiro, Uberaba, MG, Brasil, no período de 1970 a 2000. Verificou-se a prevalência da cisticercose em 71 casos, sendo 18 (25,4 por cento) de cisticercose cardíaca e 53 (74,6 por cento) de encefálica. Foram analisados 19 cisticercos, sendo 9 de encéfalos e 10 de coraçöes. Os cisticercos foram classificados, de acordo com sua fase evolutiva, em etapas: vesicular, vesicular coloidal, granular nodular e nodular calcificada, com similaridade entre os diagnósticos macro e microscópico. Entre os processos patológicos destacaram-se a beta-fibrilose e a fibroelastose endocárdica. Adicionalmente, demonstrou-se que a classificaçäo pode ser aplicada tanto na cisticercose encefálica como na cardíaca

ABSTRACT

A cisticercose é uma doença infecciosa, endêmica em alguns países, que pode acometer qualquer órgão, sendo muito rara na mama. Foram encontradas apenas 21 casos descritos na literatura no período de janeiro de 1966 a novembro de 2001. Os autores relatam um caso de uma mulher de 29 anos que procurou o serviço de mastologia com tumoração indolor na mama direita. O exame anatomopatológico da peça revelou escoléx de Taenia solium. São discutidas as características macro e microscópicas da cisticercose na mama, correlacionando este caso com os outros 21 descritos.


Cysticercosis is an infective disease, endemic in some countries, that many invade any organ, but the breast location is rare. A case of cysticercosis of the breast in a 29-year-old woman is reported. Clinical examination revealed a painless, well delimited, mobile nodule measuring 2,5cm in diameter. The clinical and histological aspects of the cysticercosis of the breast are discussed considering 21 cases described in the literature from 1966 to 2001, comparing to this new one.

In the present study, we have determined the growth and development pattern of rostellar hooklets of Taenia solium cysticerci (Zhengzhou and Harbin strains) in three pigs (1 SEM and 2 L-SEM strains) 89-196 days post experimental infection. A total of 3,675 cysticerci were collected from 3 pigs, 3,007 (82%) of 3,675 cysticerci were evaginated by enzyme method. 439 (15%) evaginated cysticerci were carefully examined and measured after dehydration, staining, and mounting on microscopic slides. Among 439 cysticerci, 234 (53%) had pair rostellar hooks, 88 (20%) with unpair hooks, 60 (14%) only small (outer row) hooks, and 57 (13%) no hooks including 34 hooks were completely dropped and 23 no hooks developed. The number ranged from 10 to 17 pairs for pair hooks and 1 to 29 for unpair ones. The length and width of rostallar hooks on the scolex of cysticerci were usually larger in the pig with longer infection time. Moreover, cysticerci with pair and unpair rostellar hooks had only small hooks and no hooks were present on their scolices. However, cysticerci with only large (inner row) hooks were not found. These findings indicate that the growth and development of small hooks precedes that of the large hooks in the formation of the two-row pattern rostellar hook in Cysticercus cellulosae.

ABSTRACT

The major three species of human taeniid cestodes, Taenia solium, T. saginata and T. saginata asiatica (= T. asiatica) which require humans as the definitive host are still not rare in developing countries. Among these, T. solium is the most serious with medical and economic importance. Neurocysticercosis (NCC) in humans is now recognized as the major cause of neurologic disease in the world. As these human taeniid cestodes obligatory require domestic animals such as swine, cattle and swine as the major intermediate host animals respectively, it is not easy to analyze the basic research in these domestic animals. In this brief review, we introduce experimental animal model for these three species in order to obtain fully developed metacestode stage in severe combined immunodeficiency (scid) mice. Non-obese diabetic scid (NOD-scid) mice are expected to be a satisfactory animal model and to have advantages for analysis by several view points of developmental biology with gene expression throughout development, antigenic homology of cyst fluid of these three species, evaluation of drug efficacy or metacestocidal drug designs, confirmation of unknown taeniid gravid segments for identification based on the morphology and DNA analysis of metacestodes. The animal model is not only available for human Taenia spp but can also be applied to other taeniid cestodes of economic importance or in veterinary parasitology.

A cistecercose humana é uma doença caracterizada pela presença de larva metacestóidea de Taenia solium nos tecidos e decorrente da ingestäo acidental de ovos viáveis, provenientes da matéria decal de indivíduos portadores do verme adulto. Dentre as possíveis localizaçöes do cisticerco, destaca-se o Sistema Nervoso Central (SNC) pelo poliformismo, pela gravidade dos sintomas e pelas dificuldades diagnósticas. O curso da doença é particularmente dependente de uma variedade de reaçöes imunes e inflamatórias desencadeadas pela interaçäo parasito-hospedeiro, que se desenvolve em diferentes graus de complexidade, desde a ativaçäo e migraçäo das oncosferas, a partir da luz do intestino delgado, até a implantaçäo e sobrevida da larva em tecidos do organismo hospedeiro. O estágio de evoluçäo, a quantidade e a localizaçäo dos cisticercos exercem açäo sobre a sintomatologia e podem influenciar nos diagnósticos imunológico e por imagem. O duagnóstico definitivo somente pode ser firmado através da demonstraçäo do parasito por pesquisa direta; entretanto, questöes de ordem técnica e de caráter invasivo limitam seu emprego. ABSTRACT

El 13 de diciembre de 1997 acudió al servicio de urgencias del Hospital Mocel una paciente femenina de 27 años de edad con historia de cefalea de seis meses de evolución. Había sido diagnosticada en otro hospital con neurocisticercosis y tratada médicamente en forma muy irregular. Dos días antes de su ingreso presentó síndrome cráneo-hipertensivo con deterioro del estado de conciencia, hemiparesia derecha y síndrome de babinski bilateral de predominio derecho. Se le practicó una tomografía de cráneo urgente que demostró lesión anular con centro hipodenso, punto pericentral y bordes de hiperdensidad mayor, localizado en la porción anterior del lóbulo frontal izquierdo, hasta la convexidad, compresión del cuerno frontal, gran edema perilesional e importante efecto de masa, motivo por el cual fue sometida a una operación quirúrgica de urgencia

ABSTRACT

A retrospective analysis of fine needle aspirates of 132 cases of cysticercosis presenting as palpable nodule is presented. In 98 cases, larval parts, detached hooklets and scolex established the diagnosis; in another 34 cases, the background inflammatory pattern was helpful in suggesting the diagnosis of a parastic lesion.

ABSTRACT

La neurocisticercosis (NC) es la parasitosis que afecta con mayor frecuencia el Sistema Nervioso Central (SNC) en todo el mundo. Casi la mitad de los pacientes con NC presenta meningitis cisticercosa, a pesar de lo cual, existe poca información actualizada con respecto a esta entidad. Esta forma de presentación puede ser de extraorinaria gravedad y por lo regular sigue un curso crónico. Hay controversia con respecto al manejo y algunos autores señalan que el uso de esteroides no reporta beneficio. Presentamos 5 casos de meningitis cisticercosa, seguidos durante un promedio de 18 meses y hacemos algunas consideraciones clínicas importantes, enfatizando la utilidad de los esteroides en el tratamiento de esta grave condición

ABSTRACT

El objetivo de este estudio fue comparar la reacción inflamatoria que causan los metacestodos de T. solium en tejidos muscular y nervioso de cerdos. Se estudiaron músculos y encéfalos parasitados de cerdos, inoculadoss con huevos de parásito adulto y sacrificados a los 70 y 210 días posinoculación. Los resultados demuestran que las larvas alojadas en el tejido muscular causan una respuesta inflmatoria más severa que las localizadas en el encéfalo. El análisis estadístico Kruskal-Wallis y Quade de los resultados, indican una diferencia altamente significativa (p=3.8 E-0.8) con lesiones más acentudas en el tejido muscular
